Rectification Spouse Name ( Not Change of Name )

Hi,
In my passport, my wife's name is written as: <First Name> <Father's Name> <Last Name>
In her passport, her name is, just: <First Name> <Last Name>

In all her records(Marks card, degree certificates etc), her name is <First Name> <Last Name>.

I want to get it rectified to: <First Name> <Last Name>, in my passport's spouse name.
Should I submit Annexure-E ( which is submitted for change in name ) for this!?
If not, then what is the procedure?

Any help, guidance is appreciated.
