I-485 for EB1A - Sample - intent to work in the same ocuupational field as I140

For EB1A petitioner filing I-485 to change status, there is a need to submit a signed statement confirming you intend to work in the occupational field specified in the Form I-140.

Does anybody have such letter sample or example?

I would like to submit letter with proper language and wordings.
Any help would be greatly appreciated.

I am in EB1A category - I didn't have to do this.... where is that required on the form (unless it is new)?
