Alien Name on Labor app

Hi All,

I really need clarification on this minor but dunno how important detail.
I have been given the Labor app that will be filed to verify, my first name is Jack and middle name is Samuel and I see that in the alien Name section, in the document under the first name its spelt "Jack Samuel" and under middle name its blank... there is a clear distinction but would this really matter?

I really need your help in doing the right thing, anyone face this problem?
