Employer's Declaration for Schengen Visa

Hi,

As you all know the we need a employee declaration to apply for Schengen Visa. I came across through various templates but all of them were for ONE country only. My question is that what if I am traveling to 3 countries then how the template should look like - please see the text enclosed within hash (#####) for which this question is being asked.
