Schengen Visa Application Question

Dear Sirs:

A relative is sponsoring my trip to Germany. During my stay, I plan to travel by train to Paris (from Munich) stay there for 3 days, then go back to Munich, take the train to Rome and stay there for 3 days. Then I go back to Munich. My then are questions are:

• In my application for a Schengen visa, do I need to apply for a multiple entry if I plan to go to France and Itlay?
• Do I need confirmed or paid reservation to hotels in Paris and Italy when I apply?
