Stayed out of Country more than 5 month on Stem OPT

I am out of the country from September 2021, but I have a valid Job and a residential address in the USA and submitting my stem opt reporting on time to my DSO and I could see the same in my SEVIS portal. I planned to come to the USA in December 2021 but I couldn't come back for some personal reasons...

I was an international student and during 2017-18 my grade had been poor so I was academically dismissed. I left the US in mid-September. The school readmitted me for spring2019 and had issued me a new I-20.
So my question is: Do I need to get a new F-1 visa? My previous one is not expired till...
