Need Help with the PIO Order Form on BLS website & Airway Bill Number

I am trying to fill the PIO Order Form on the BLS website. For the "Application Type" if I select "Postal" I need to fill in Airway Bill No. Similarly if I select "Prepaid Delivery" to "yes" I need to fill in the "Return Envelop Airway...
