I am in process of filling BLS application for Name split in Passport in Indian passport.
Current My name in Passport is

SURNAME : BLANK
GIVEN NAME : MOHAMMAD AFZAL

I would like to get it changed to
SURNAME : AFZAL
GIVEN NAME...
