H4 221-g - Yellow Slip - Status - Refused - Admin Processing - Refused

My wife got an 221-g yellow slip for H4 visa in New Delhi. Visa officer retained her passport. I have been watching status since then. Initially status showed as Refused and next day it changed to Admin Processing. Then, the next day, it changed again to Refused.
