New OCI application Self Attestation of copies

With a new OCI application, when you submit the Notarized Affidavit in Lieu of Originals, do you still have to individually self-attest every copy with "I attest this is a True copy of the original" statement?

I am currently on my F1 visa and applying a F2 dependent visa for my spouse. I am going to be the official sponsor as well(providing my USA bank statements as financial proof). Do I need to provide the Form I-134 for her visa interview? Please let me know.
