H1B visa insurance

Do I need to have medical insurance if I travel on h1b-visa?
If yes, what kind of insurance it should be? Is there a guideline on that?

I currently hold H1B Visa, but my employer has not provided me any medical insurance. As H1B holder, do I have right to ask for the medical insurance from my employer?

This might be a repeat question for this forum but I could not search for the information. Thanks for any suggestion...
