Tricky situation - need info for h4 to h1b transfer cos

1. In 2017, my wife who was residing in US on H4 visa, traveled to India on March 29th and returned back to US on H4 visa in July.

2. The same year, 2017, Employer X applied for her H1B....

I have an example scenario below for which I would need an answer.

A - Has H1 with Employer X.
B - Spouse of A on H4 with Employer X.

B is filing for H1 and A can do H1 transfer with new Employer say 'Y' simultaneously?

During H4 to H1 transition processing,Can the H4 travel go out of country and come back to US with the same H4 while the newly filed H1 COS is under processing.
Do we find any issues at during port of entry when we travel back using the existing H4 ?
