DS 160 - Previous US Visa

I am applying for a B1 / B2.

I have 2 US visas - a L1 (active and latest) and a F1 (expired).

I noticed that there is provision to enter only 1 Visa's details even though the wording says "Previous US Visas".

I have a question on H1B extension.

I previously worked on L1 status between 2003 and 2007 for a period of 37 months (after removing the gaps between my visits to India).

In 2006 I applied for H1B and got the approval in Jan 2007. But I did not switch to H1B immediately...
