Is my B1 Visa Rejected twice because of my travel history?

Recently i have applied for B1 Visa and it was rejected twice in Hyderabad.

First attempt Hyderabad Oct 23rd 2018:

Do I need to worry about missing and incorrect entries that I see on my I-94 electronic record?

For example, on one line, I see incorrect year, and one travel history has my start and end date the same!

Hi,

I got my B1/B2 visa with 3 months validity single entry but somehow my travel to US has been cancelled due to some business decisions (priority changed). now if I don't traveled to US and visa get expired does this affect in future for getting the Visa (for B1 or any other category)....
