Wrong Surname in my I-797 document (Approval notice) :(

Hi All,

my employer submitted H1B Amendment in July and got my approval documents.

But my Surname is wrongly spelled in I-797 and Form I-129 documents.

Actually there was a correction in my surname in passport and correct surname is in Second page. But...

For CAP 2016, My petition is approved . But I realized now that my name shown on petition form and my name on my passport are not matching.
