Declaration form for PIO application

One of the required documents in checklist for PIO application (for minor as well as adult) is Declaration form.

If the application is for infants, do we have to put their thumb impression or infant's parents need to sign this form?

There are no instructions available for CKGS Declaration Form, CKGS Disclaimer Form & CKGS Order Form on the website. I too have the same question. Whether these forms need thumb impression for Minor or Parents need to sign it or only father should sign it? Also shall these be notarized or not?

I came to know that parents can sign the declaration form for a minor. It is better to take thumb impression of minor on the form, parents can sign below the box for applicant's sign/thumb impression.
