Procedures/documents required for H4 visa application

Hi all,

My boyfriend is currently on H1B visa whilst I only hold a B2 visa. We plan on going back to our home country to get married soon. After the marriage I plan on applying for the H4 visa so that we could return to US together.

I was wondering what are the procedures required to apply for the H4 visa and what are the documents required? Does his form I797 needs to be amended?
