I would like to seek for advise regarding the part of the form that stated US Point of contact information (a person or anyone who can verify identity). I have a relative in California but i am planning to go to New York & stay in a hotel as i will visit US for 8 days only. I want to know if i should fill the form the address of my relative or the address of the hotel i will be staying. Appreciate any kind of advise please. Thank you.
