This post relates to B1/B2 Visitor Visa only.

Mine and my wife's passports don't have Surnames and look like:
Myself: Surname - (Blank) Given name : AAAA BBBB
Wife: Surname - (Blank) Given name : AAAA BBBB CCCC
DS-160 was filled as per instructions:
Myself: Surname - AAAA BBBB Given name : FNU
Wife: Surname - AAAA BBBB CCCC Given name : FNU
Actual visa received from US Embassy:
Myself: Surname - BBBB Given name : AAAA
Wife: Surname - CCCC Given name : AAAA BBBB
I had correspondence with their representative about the difference in DS-160 Form and Actual Visa names to avoid any problem at US Immigration. Their reply is reproduced below:
"Please be informed that even though the passports do not have surnames on the field but because you have more than 1 names, US Embassy put the last name as the surname now".
Update: We didn't have any problem at US immigration.
Assumption: So it means applicant may fill the last word in Given Name in passport as Surname. Advise to check and re-confirm.
Air-Tickets:
1. International Travel: Used Qatar Airways and they advised to use names matching the US Visa.
The names on the tickets matched the US Visa format. i.e.
Myself: Surname - BBBB Given name : AAAA
Wife: Surname - CCCC Given name : AAAA BBBB
Absolutely no problem in travelling and at immigration.
2. Domestic travel: Used United and American Airlines. Both insisted on using the names exactly as on the passports.
So the domestic tickets looked like:
Myself: Surname - AAAA BBBB Given name : FNU
Wife: Surname - AAAA BBBB CCCC Given name : FNU
Again, no problem in domestic travelling.
Again, before proceeding please verify and re-check the requirements.
