Transit Visa Application to London

Hi,

I'am Anil Chopra. I had received my H1B (I797) Approval Noice. I have to get it stamped in Chennai, India, for converting this notice into H1B visa. In order to get it stamped, I plan to go to Chennai, India, on April 28, 2006, via
London.

I started the online application for DAT visa but in order to
complete it, I have couple of questions.

First:
I'am going to India after leaving London. The online questionnaire
asks for visa or permit number for entering India. Since I'am an
Indian National, I do not need visa or permit number to enter India.
What should I enter in these fields? Should I enter my passport
details instead?

Second:
I'am returning back to US on May 28,2006, via London. I would have got
my H1B visa by then. But the online questionnaire is asking for those
details now. In my inbound section, the question is "do I have visa to
enter US" which I don't have now but would be having after I get it
stamped in Chennai on May 1, 2006. So, please let me know what should
I enter in these fields.
