transit visa for London

hi..
am planning to send docs british consulate for transit visa as my journey is to india via london...and my h1/h4 visa got expired but have renewal papers..

so applyinhg for Direct airside transit visa..with following docs..

1. online filling of applicaiton
2. passport
3. h1 visa renewal original
4. passport photos 2x2
5. itenary
6. fee will pay via credit card online or MO

will send above docs by post to New york as i stay in CT..

do i have fill the online app seperatly for wife h4 too?
pls. let me know if the above docs are enogh for sending via post..
