Travel Documents for Tourist Visa ( Parents )

Hi,

I had applied for tourist visa for my mother 2 years back and thankfully she got it stamped without any issues.
She could not travel due to some personal problems.
And I have booked tickets for her this 15th April , I did not have the patience to check what documents she needs and assumed it would be normal ones or the same file she took for stamping.
Could somebody tell me what documents does she need for the travel.

I read that, it says visa stamped passport that's it. Could you please confirm.

passport with visa and contact info about the host, just in case they ask more about who she is visiting.

travel health insurance is NOT mandatory for admission but it is recommended for personal benefit as out of pocket medical expenses in US are very high.
