Spouse name in visa application

Hi,
I am filling the visa application and in there it asks for spouse name ( it also says to include spouse maiden name)

Our marriage certificate and my wife passport holds her new name ( married name)
while her birth certificate and other docs has her maiden name.

My query is what should I fill

eg: suppose
maiden-surname - 'B'
first name - 'A'
married-surname - 'C'.

So, her name before marriage was 'A B' ( as in her birth certificate) and after marriage is 'A C' ( as in our marriage certificate and in passport)

So, what name should I fill in application.
1) Surname : C
Given name : A

or

2) Surname : C
Given name : A B

Please suggest.
