In the "Spouse's Full Name" section, list both names to cover all bases: Trina Bhattacharya Kar. For "Surnames," include both surnames: Bhattacharya and Kar. For "Given Names," write Trina. This ensures inclusion of both maiden and married names for clarity in documentation.

Filling Up Spouse's Full Name (Including Maiden Name)

For the section "Spouse's Full Name (include Maiden Name)", you should fill it out as follows:

        Spouse's Surnames:
        • Kar (Current surname)
        • Bhattacharya (Maiden surname)

        Spouse's Given Names:
        • Trina (Given name)

It's crucial to include both surnames to accurately represent her identity and history. Some forms may provide separate fields for "Last Name" and "Maiden Name," while others may require you to include them both in the same field. In the latter case, you can separate them with a comma or hyphen:

        Spouse's Surnames: Kar, Bhattacharya

        Spouse's Given Names: Trina

Previous posts may have caused confusion due to differing formats and regulations across various forms and applications. However, by following this approach, you can ensure accurate and complete information regarding your spouse's full name, including her maiden name.

If you're unsure about the specific format required for a particular form, you echatrandom can always contact the relevant agency for clarification.

For "Spouse's Surnames," you can input both her maiden and current surnames. So, you'd include both "Bhattacharya" and "Kar" in this field, separated by a comma or a slash ("/") to indicate the change in surnames due to marriage.

For "Spouse's Given Names," enter her first name and any middle names, if applicable. In this case, it would be "Trina."

Here's an example of how you might fill it out:
          • Spouse's Surnames: Bhattacharya/Kar
          • Spouse's Given Names: Trina

Thank you for the clear instructions. I'll enter "Bhattacharya/Kar" under "Spouse's Surnames" to reflect both her maiden and current names, and "Trina" in the "Spouse's Given Names" section. This format effectively communicates the name change while maintaining accuracy.

In the "Spouse's Full Name" section, you should enter "Trina Bhattacharya Kar" to include both maiden and married names. Under "Surnames," list "Bhattacharya" and "Kar," and for "Given Names," simply write "Trina" for clear and comprehensive documentation.

When completing the DS-160 form for a U.S. visa application, it is important to accurately include your maiden name , if applicable. In the "Personal Information" section, you will find a field specifically for this purpose. Enter your maiden name as it appears on your official documents, such as your birth certificate or previous identification.
