B1/B2 refused - medical examination requested

My visa interview went well and the officer told me that my visa was approved and that i would receive my passport in a few days. When i went to collect my passport, it had a letter stating visa was refused and that i had to undergo a medical exam and submit the results. I am wondering as to why medical exam is being requested for non immigrant visa. I have lived in the US for 14 years and relocated to India 4 years ago. I did not have a green card at any time.
