    1. While filling up DS-160 form, there is a question which says - "Have you ever used other names (i.e., maiden, religious, professional, alias, etc.)?" (see below snapshot)Click image for larger version

    To give my background, I did not have my surname earlier. I had only my first name(Ex:- ZZZ). I had got my surname(Ex:-YYY) added few years back, and also got it updated in my passport. Therefore, mu full name is 'ZZZ, YYY'.
    As I did not have any surname earlier, for the above mentioned question, what should I be filling under surname field? The field cannot be left blank in the form. Please suggest a proper way to fill it up.


    2. In the form, we are asked to fill spouse's name along with maiden name. But there is only single field to enter the surname. Should I be writing the current surname as well as maiden name in the same field? If so, what is the correct format/order?

      1. Surname would be the YYY.
      Given name ZZZ
      Full name ZZZ, YYY

      b. Have you ever used other names. If you say yes, then put in that name, could be a popular nickname that you go by, otherwise put NO.

      2. Put her surname and her maiden name on the same line
      Then her given name below

        DS-160 form - spouse surname added recently in passport

        Even I am facing the same issue - my spouse did not have Surname in earlier passport and now have got it changed to add Surname.
        Any updates on how to fill DS-160 in this case?

          Not sure what is the issue here.

          Earlier you had just first name, now you've both.

          Your DS160 should match with your passport.

          In earlier name used - you can provide just your first name.
