1. While filling up DS-160 form, there is a question which says - "Have you ever used other names (i.e., maiden, religious, professional, alias, etc.)?" (see below snapshot)

To give my background, I did not have my surname earlier. I had only my first name(Ex:- ZZZ). I had got my surname(Ex:-YYY) added few years back, and also got it updated in my passport. Therefore, mu full name is 'ZZZ, YYY'.
As I did not have any surname earlier, for the above mentioned question, what should I be filling under surname field? The field cannot be left blank in the form. Please suggest a proper way to fill it up.
2. In the form, we are asked to fill spouse's name along with maiden name. But there is only single field to enter the surname. Should I be writing the current surname as well as maiden name in the same field? If so, what is the correct format/order?
