To whom it may correspond,
During 2008, I was -up tp now- 3 times for Business resons in the USA, being the last one during 14th-18th June. On that trip my Form I-94 W was not collected, but I did not pay much attention as I was not aware about the obligation to return it.
I am a German citizen working in the Frankfurt area and today I contacted the Consulate in Frankfurt, so they informed me how to proceed.
Please find attached the following documents:
- Copy of my passport
- Copy of my Itinerary (flight details)
- Copy of the I-94 W
I have to check if I still have my bording passes.
All these documents are on their postal way to the following address:

Please let me know any further procedure that I need to follow to sort this issue.
