    Hello,

    I am trying to find out how does one get proof of name change that was done during their naturalization process. My wife changed her name at the time when she was naturalized. The interviewer or person at the time ask her what name she wanted and it was changed as showed on her Certification of Naturalization but she never got any kind of letter or proof that shows what her old prior name was and the new current name is. The reason I'm looking for this proof is because my company's insurance company wants to make sure she is who she is since her name on our marriage certificate is her old name before she got naturalized and changed her name.

    That's odd. Was this a judicial oath (at a later date at a courthouse)? If so, she ought to have been given a name-change form. Contact the court. If it was an administrative oath (usually a same-day oath), then make an Infopass appointment with CIS and talk to them about it.

      You can request copy at immigration office or court where oath ceremony took place. Name changes are usually granted by judge so there has to be public record for that.
