  • US Passport authorities query -URGENT

    II (father of the kid) do not have last name in any of my records/documents. But I always wanted that my child should have last name. Let’s assume our last name is “Bdi”. We got the birth certificate of our child from the authorities. Everything is proper in child’s name. The birth certificate had “Bdi” as last name against father’s name as well. Note that father has never used any last name in any of his documents.

    We applied for passport for our kid. We received a letter from the passport agency as follows.

    “The relationship between you and the parent signing your application has not been established. Your father’s name “Bdi” as listed on your birth certificate is unaccounted for. We are unaware how your parent acquired his current name. Please submit evidence of a legal name change in the form of a marriage certificate, divorce decree, or court order showing how your parent acquired his current name.”
    I tried contacting the passport agency customer service. They said the agency will contact me. No one has reached out to me till now.
    Now the question is:
    1) I do not actually have any name change. It’s just that the last name was added against my first name in my kid’s birth certificate. My marriage certificate also does not have my last name. Changing my name would be a very lengthy process and I would not like to go through it.
    2) Do the passport agencies accept any affidavit signed by me stating that the last name”Bdi” was inadvertently added against my name?
    3) Would it be possible to correct the birth certificate and submit the corrected certificate to the passport agencies?
    4) Any other options available? Please provide me suggestions.

      You can reach out to the Passport office Call Center number and tell them to arrange for a call with the respective National Passport Center where your application of passport falls depending on the state you live in. The link given below may provide you help with respect to the region or you may ask the call center representative as to what would be your regional passport office.

      The National Passport Center guys will call you based on the query that you have registered and will tell you exactly what they need to correct it. If there was an error in birth certificate of the child, then you might have to get it correct from the Birth Records Office and take a new birth certificate with all correct details and send the passport office the new birth certificate. They will accept it. They would return the original birth certificate once the passport application is processed.
