Employment address while on consulting assignments

Hi,

Can anybody please help me on question 3 (Employment or School Name). I am doing consulting for last 5-6 years and worked mostly on client office only. Whenever my contract ends I find new W2-staffing employer and work for his client. So I am wondering whether should I put the client-name and address in the employment section or should I put my W2 employers and his offlice address?
