  • Name Change Certificate

I misplace the "name change certificate". I would like to know where can I apply for a copy? I change my name through naturalization.

You can try asking for an appointment via infopass. I am in the same process I still have my old name, I got married and my last name suposed to be different, I asked to them and that's what they told me to do.

I also have the same problem. I can't get copies of my school transcript because they don't believe i use to be that person. Did this work for you by contacting infopass.

I'm trying something else right now because I feel all alone in this matter, but now that I found someone else in my same situation we can help each other out. This message board isn't very helpful cause I wrote a message like in september and had 0 response. Right now I'm using my Freedom on Information Act to obtain as much information about me from the government as possible which includes naturalization record and hopefully my name change document will be in there. If it isn't hopefully, it will have something that has my old name and new name together. If you have any other solution, let me know and I'll keep you updated as well.

You can go to the local court where you have obtained it first time. You might have to pay the fee for getting the additional copies.

that's the problem. When you get it done through the naturalization process, it is done through them. I had my copy given to me at the oath along with my naturalization certificate. I went to the record archive and they can't find it without a case number, which doesn't exist because i never went through court.

Today I tried the infopass way as well and it seemed to be working. I don't have the paper yet but the immigration officer said they have ordered my files and to come back in about 3 weeks to pick up a copy. They said it is one of the more common problems they see. Hopefully this will help.
