My wife had some self employment income for couple of months (about 4 ½ years back) and filed the taxes accordingly. She did this from home. She has been a home maker otherwise. Does she need to enter this in the section (6B) asking for work/employer information.

Yes, if she has filled taxes it is all on record (IRS) and she has to mention it on the application.
