OCI (India Visa) - Middle name on Indian Passport, no middle name on U.S. passport

I have my wife's middle name missing/not mentioned on the US passport and also Naturalization certificate . When she went for oath ceremony it was not required absolutely to mention so she just mentioned her first name and last name. However now when doing OCI application they rejected the application stating that we need to submit notarized statement for removing middle name on USA passport

Could anyone point out what kind of statement should I provide here. It was just left out as it was not required as absolutely necessary to mention the middle name when doing the US passport / Naturalization certificate

Can you post a screenshot of the email received from VFS. I believe it's the same thing that happened to us but it was regarding last name aka family name fory father. Then I'll let you know what we did to fix it.
