need help - citizenship question

Hi,
I was interviewed last month for my citizenship test. The officer said the decision can not be made until they verify my husband's employment. Thye sent me a letter after two weeks to come back with a few papers. They are asking me to bring the notarized document of addresses I have lived in US, my husband's employers details.
My question is ...how do I document to notarize the addresses? Can someone help me get a sample of document?
