N-565 Address for Evidence

Hi everyone,

I recently change my name and would need to replace my certificate of naturalization. I filed the form online and sent the original evidence to the Nebraska Service Center (as instructed on the form).
My package is returned to me due to "Forward expired", which means I can't send my package to that address. I've called and checked on all USCIS sites but they are not very helpful information. Can someone please let me know if the below address is correct? I live in CA.
