  • Filling out the N-400 form

    Hi. My girlfriend is filling out her N-400 form for naturalization. There's a few contradictions between the form, the guide, and online information.

    1. The areas that we do not fill out, do we leave them blank unless the guide says otherwise, or do we always write "N/A" or "NONE"?

    2. When stating her addresses for the last 5 years, the guide says to write MM/YYYY and the form says MM/DD/YYYY. Does it matter which one we use, or if we use MM/DD/YYYY for the ones she can remember, and MM/YYYY for the ones she's unsure about?

    3. Name change. She would like to keep her first name, but change her last name back to her maiden name. Is this something that might delay the process? I presume she would need to get her other ID's renewed after the name change, but would her new Citizenship ID be enough to get a passport, or would she need to get a new Driver's License or State ID first?

    1. I used either N/A or left them blank.

    2. Make your best guess and use MM/DD/YYYY

    3. Yep, the best route is first to renew DL and then apply for the passport. The name change does usually cause a delay in the N400 process (since the oath is done at a court house I guess) but it should not be that much. Maybe someone on the forum who has gone through the name change process can comment on that.
