OCI (India Visa) - Middle name on Indian Passport, no middle name on U.S. passport

    Hello,

    For my wife's application for OCI, we have a mis-match in middle name. Her last Indian passport had my name as the middle name. However, during the naturalization process, the immigration officer suggested that husband's name could not be used as middle name, so my wife asked him to drop the middle name completely (she only has a first and last name). There were no court affidavits, etc that were filed for this, since the officer considered it an existing legal name.

    For OCI application, unfortunately, this seems problematic - since the application guidelines state that the names must match, otherwise a proof is required suggesting why & how the name was changed.

    Can someone suggest what I should do in this situation? I can get a notarized statement that her middle name was dropped, but I do not have a reason that I can state. If someone has a sample statement that I can use, it will be helpful.
