Applying for a J2 EAD (previously held other EAD)

Hello folks,
I am applying for a new J2 EAD and had some questions about the I-765 form.

I was previously on a F1 OPT and subsequently on a H1-B visa have applied for an EAD (and as such, possess an alien number)

I'd like to know if this application form would constitute:

1.a. Initial permission to accept employment
OR
1.c. Renewal of my permission to accept employment. (Attach a copy of your previous employment authorization document)

Would it make sense to also include my previous (F1) Alien Registration Number (A-number)
