Maiden vs married name

After successful interview the consular gives the package to surrender @ port of entry In USA.
Do they print married name on visa package envelop or the maiden name on passport ???

was your fiance(e) married?

If you're applying for a K-1 visa, then you technically shouldn't be married.
Was your fiance(e) married before?
