Paperwork and Med Exam for K-1

I was just wondering when does the medical exam for the K-1 need to be performed? Does it need to accompany the I-129F paperwork to the States or is this something that needs to be done before the K-1 is processed in the originating country.
Also, does the police record paperwork need to accompany the I-129F as well?
