2. Do I need additional form with this 1-129F
3. Do I really need an attorney to do the filing for me. ?

If you were born in the US, a birth certificate showing birth in the US or a valid US passport.

If you were born abroad and were a citizen at birth, either a CRBA or a valid US passport.
