Thanks for all the previous responces. One more question, does my birth certificate need to be certified? I would just go get a copy but lost the one I had in the Katrina Flood. Obtaining a new certificate is very painful, taking over 8 weeks. I have a regular copy, but fear this isnt enough. Thanks again for the help.

Why don't you send in the regular copy with the paperwork now, to get things started. And at the same time, you apply for a new certified copy which - asyou wrote - should get to you in about 8 weeks. That way, if they ask for a certified copy later, you will be able to show it, but right now you will avoid further delay.
