Is this an actual form or just a letter you craft telling how you and your future spouse met? If it is a form, can someone tell me where to find it?

Thanks for your answer. However, I was talking about another form that complements the I-129F form, where you can explain how you met your future spouse. I reas so many mentions fo a supplement for for question 18 on the I-129F and I want to know if there is a form for it or if it is just your testimonial printed on a separate piece of paper. Do you know?

It seems in most cases, people just write up a paragraph explaining how you met, etc. etc. It is not a form, it is just something you add (if you run out of space in that box).

There is no "template" just put in the information asked for.
