Question about for FC-029

Can you please tell me what is form FC-029 because i couldn't find on INS.GOV form list? is it needed ? do we both need to file it?

It's basically a form that you sign that states you can and will send unaltered photo copies front and back of all originals. It also states you must be prepared to present originals if required.
