I wanna ask about the form G-325A,,I fill out but i confused,because saw in the example had fill out already,,it will say (family name) like father and mother "maiden name of mother",then i saw same as surname..I ask,,what should I put to maiden name of my mother? I will put my middle initial from her? or her surname from my father? like same as surname...i hope I can hear you soon,thanks and God Bless..
