Changing visa type while in USA

I am trying to find out how to change a visitor visa to a fiance visa or a spouse visa while still in the USA. My fiance came to the US in December on a visitor visa from South Africa and we are trying to see if it can be changed while he is still here. What steps should be taken and when is a good time to inquire?
