UPDATE:
yesterday, august 25, 2009, i had my medical check-up at St. luke's clinic in manila, philippines. i arrived there around 5:30am and wow i'm 73rd on the line...really most applicants go there so early...
anyway, here's what happened.
before 6am the security guard gave us a number and forms for the check-up. he let us go inside the building and fill-up the forms there. BTW, they look for 4pcs visa pics, photo copy of the notice from the embassy and the appointment letter for the interview.
7am they started to call by numbers, first for picture and finger printing. then next is for the data encoding, all in the first floor of the building.
next is payment at the 5th floor. then x-ray at 3rd floor. then laboratory in 4th floor where they will take blood sample. they didn't took my urine sample since its for preg test only but i noticed some guys also gave urine sample. after that is the physical examination at the 5th floor where they will check the eyes, height and weight and the vital signs. then a doctor will call you in a cubicle for further check and will let you disrobe...
next is to proceed at the check out counter in the 2nd floor where i left all the forms. they gave me the receipt and claim stub and my passport and told me to come back the following day for they result and further instructions.
that's what happened the first day, i finished around 12:30noon time, i started 73rd on the line.
next day, august 26, 2009.
i arrived 9am at the clinic and gave my receipt and passport at the guard in the lobby ground floor. he checked my files and told me i passed the exam, i'm now good to go for immunization. he then marked my receipt "for immunization" at the 2nd floor. i waited for about 15mins before my turn. they gave me 2 shots. after that, they sent me at the ground floor and wait for my name to be called, around 15mins, i was called again, let me sign 3 forms and asked me to wait again. 20mins after, i was called back and gave me the cd and immunization record. i left my medical result and passport at the clinic, they'll be the one to forward it in the embassy for my interview.
now i will prepare for my interview on sept. 15, 2009. wish me good luck folks...cheers everyone!

If everything goes well... ( already bought his ticket... )
Btw, We were able to choose what date to go for the interview and his medical exam is walk-in basis first come first serve. We didn't have to wait for him to receive the notice because the call center operator said that it's just a notice via mail that they received the packet and he is eligible to set an appointment for interview and that packet 4 is printed online.. He just needed to show the confirmation email stating the interview date that he printed out for his medical exam instead of the notice.
My Fiance's Flight to California (LAX): 10/16/2009

just for the update...
yesterday, sept. 2, 2009 i payed my visa at the bank and they gave me a validated DS-157 form. in the website, it says i need to submit 2 copies of the DS-157 so most probably the other form is not validated by the bank, would that still be ok? anyone got idea on this matter?
i'm still on the process of gathering other documents for my interview, soon everything will be ready. good luck to everybody...
