Police reports, Residency, Embassies?

My situation: I'm a US citizen, my girlfriend is from Uzbekistan, but resides in the U.A.E. on a work visa.
Questions
1.Police Reports: US visa website says police reports are not available for Uzbekistan, does that mean she doesn't have to provide one?
2. Names: My girlfriend was born to Russian parents, which use a different name format than we do here in the US: First, Paternal last name, then Maternal last name, how do we enter names on the forms?
3. Residence's: My girlfriend doesn't have a record of the addresses of the different apartments she's lived in since she's been in the U.A.E., is this a problem?
4. Visa Interview: Since my girlfriend is a citizen of Uzbekistan, but works and resides in the U.A.E., which consulate or embassy would she have to use, one in the U.A.E. or would she have to return home to Uzbekistan?

I am sure she can apply at Abu Dhabi embassy in UAE since she has a work visa there.
