G-325A Assistance

I was wondering if anyone could help me with questions regarding G-325A.

First of all, I am divorced but kept my ex-husband's last name, instead of changing it back to my maiden name. Should my "family name" be my maiden name or my current last name?

Also, at the bottom of the application, it says "Print your name and alien registration number in the box outlined by heaveer border below". Since I am a United States citizen, do I fill that out? Thank you so much!

G325A Assistance

You should use the name that you go by or you could use a hyphen and use both names Jones-Smith. This is just my opinion of one way to do it.

As for the Alien registration, I wrote n/a (not applicable)
