My fiance lived in Greece - in Crete and Cyprus. Does he need one certificate from Greece or one from the city where he lived in Crete and one from the city he lived in Cyprus. I cannot find this answer anywhere! Thanks in advance . . .

He could require two certificates, one from Greece and one from Cyprus, depending on his age and length of time that he lived in each location.
From my experience, and without knowing specific details of his time spent in each location, I would say go ahead and request both.
As you said lived, I assume that he no longer lives in either location. If that is the case, and assuming he is not a Cypriot:
- Non-Cypriot citizens who resided in Cyprus in the past can obtain a "Certificate of Good Character" through the nearest Cypriot embassy/consulate
- If applying from outside of Greece, both Greek and foreign nationals should apply to the Greek Embassies at their place of residence and include in their written application full details of their identity & residence in Greece.
